When you submit the appointment request form, we collect your name, phone number, email address, reason for visit, and any notes you choose to add. We do not collect payment or insurance information through this website.
Information submitted through this site is used solely to schedule and confirm your appointment, and to follow up if we’re unable to reach you. We do not sell or share your information with third parties for marketing purposes.
Any clinical or health information shared once you become a patient is handled under our HIPAA Notice of Privacy Practices, provided separately at your first in-person visit, which governs medical record handling rather than this website policy.
